Warehouse Accuracy: 99% Per Step, 93% Overall

Supply chain manager investigating why strong individual process performance produces lower final order accuracy
99% at every step. Why only 93% at the end?

A warehouse running 99% accuracy at receiving, putaway, picking, and packing sounds pretty tight. Four steps, four nines. But if you actually multiply those out — 0.99 × 0.99 × 0.99 × 0.99 — you land at 96%. Add a fifth step like a transfer or a label scan and you're in the low 90s. That's not a disaster scenario. That's a normal operation with no single obvious failure point, bleeding out slowly across every shift.

This is why 'our counts look good' and 'our orders are going out wrong' can both be true at the same time. Each team hands off something that's 99% right and feels fine about it. Nobody's lying. Nobody's being lazy. The error just lives in the gap between steps, not inside any one step.

The fix isn't to demand 100% everywhere, which is neither realistic nor a useful target to manage toward. The fix is to figure out which step in your sequence has the highest error rate and attack that one first. In most DCs it's not receiving and it's not shipping — it's the middle. Putaway discrepancies that don't surface until a picker goes to a slot and the quantity's off by two. That two-unit gap on a 200-unit pick doesn't feel like much. Across 400 orders it becomes a customer service problem.

Counting more frequently in the right places matters more than counting everything at once. Slot-level cycle counts on high-velocity SKUs, done consistently, will tell you where your compounding starts. That's the number worth obsessing over — not your overall inventory accuracy percentage, but which specific step is doing the most damage.
